Job summary

Providence is seeking a dedicated Registered Nurse Care Manager to act as the central link for a defined patient population, coordinating with multiple PCPs to deliver holistic, evidence-based care. You will assess, educate, and connect patients with community resources to improve self-management and health outcomes.

Based in Everett, WA, this ambulatory role emphasizes autonomy within nursing standards and ethics, with opportunities to lead and grow while supporting patients, families, and care

Qualifications

  • Must hold a Washington RN license.
  • At least 2 years of RN work experience.
  • Experience with ambulatory care is preferred.

As a Care Manager RN, you'll be at the heart of transforming patient care — serving as the central link for an identified patient population and collaborating with multiple PCPs to deliver coordinated, whole-person care. You'll bring your expertise in assessment, education, and community resource coordination to empower patients in building self-efficacy and management skills, helping them achieve optimum functional health status and an improved quality of life. In this dynamic role, you'll partner with patients, families, and care teams to deliver high quality, evidence-based, cost-effective, and patient-focused outcomes. Working with a meaningful degree of autonomy within established nursing standards and the nursing code of ethics, including Chapter 18.88 RCW, you'll have the opportunity to lead and grow while performing a full range of ambulatory care RN duties.

Required Qualifications
  • Coursework/Training: Registered Nurse
  • Upon hire: Washington Registered Nurse License
  • 2 years of work experience in an RN role
Preferred Qualifications
  • Experience managing complex patients in an ambulatory setting
  • Medical Home Model experience

The amounts listed are the base pay range; the Pay Range is $52.26 - $81.13. Additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
