Job summary

Providence is seeking a Nurse Case Manager at Swedish First Hill in Seattle. In this role you coordinate care for patient populations through the acute care episode and across the continuum, working with interdisciplinary teams to improve outcomes and efficient resource use.

You'll lead care coordination initiatives, ensure compliance with regulatory standards, and partner with clinicians to deliver patient-focused, high-quality care in a compassionate environment.

Qualifications

• 3 years of Registered nursing experience in an acute care hospital.
• Applicants without a BSN must pursue and attain a BSN within three years of hire.
• Washington Registered Nurse License required at hire.

Required Qualifications

• Bachelor's Degree: Graduate of an accredited Bachelors Nursing degree program or completion of an accredited Nursing program with a bachelor's degree or higher in a health care field, Or
• Applicants without a BSN degree must actively pursue and attain a BSN within three years of hire. Continued employment by Swedish beyond three years is contingent upon attaining the BSN degree.
