Job summary

Blue Cross Blue Shield of Michigan seeks a Care Manager RN to lead a multidisciplinary team delivering holistic, person-centric care to a diverse health plan population. You will coordinate through telephonic and digital channels, assess and implement care plans, and partner with members, providers, and community resources to optimize outcomes.

The role requires active Michigan RN licensure, 3+ years clinical nursing, and 1 year in managed care.

Qualifications

• Nursing diploma or nursing degree required.
• CCM certification required or will be obtained within 18 months.
• CCP certification preferred.
• 3 years of clinical nursing in acute/post-acute and community settings.
• 1 year of managed care case management preferred.
• Michigan RN license active and unrestricted.

Responsibilities

• Lead a multidisciplinary team to provide holistic care to members telephonically and digitally.
• Use case management to assess, develop, implement, monitor, and evaluate care plans.
• Assess member health, psychosocial needs, and support systems.
• Engage member/caregiver to develop an individualized plan of care and address barriers.
• Arrange resources to meet identified needs (community resources, mental health, substance abuse, etc.).
• Coordinate care among providers, community agencies, and family.
• Advocate for members and promote self-advocacy.
• Deliver education on health literacy, self-management, medications, and nutrition.
• Monitor and reevaluate care plan effectiveness and adherence.
• Document interactions to support care management.
• Prepare discharge and post-transition care planning.
