Providence St. Joseph Hospital Eureka in Eureka, CA, is seeking a Care Manager RN for a full-time day shift.

The role involves coordinating care for complex patients, communicating with medical staff and resources, and ensuring appropriate utilization of resources across the continuum of care. Requirements include an RN license in California, an associate degree or nursing diploma, 2 years acute care experience, and 1 year in care management or related program; preferred BSN and CCM/ACM

Care Manager RN at St Joseph Hospital in Eureka, CA. This position is Full-time and will work 8-hour, Day shifts.

The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. Communicates on a daily basis with medical staff, hospital personnel and outside resources to ensure the appropriate utilization of resources according to clinical and financial standards. Monitors the medical record for variances to clinical and financial standards and intervenes to achieve the optimum outcome for the patient and the hospital.

Required Qualifications

• Associate's Degree in Nursing or a nursing diploma upon hire.
• Upon hire: California Registered Nurse License.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review in any setting, OR successful completion of TIPS program or Case Management Orientation Program upon hire.

Preferred Qualifications

• Bachelor's Degree in Nursing or higher within 3 years of hire.
• Certified Case Manager (CCM), Or
• Accredited Case Manager (ACM)

Pay Range: $54.61 - $92.23

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
