POSITION DETAILS: This is a remote/work from home position with 25% travel within Cook County, Illinois, supporting our YouthCare/Foster Care program. Position Purpose:

Develops, assesses, and facilitates complex care management activities for members with primarily physical needs to ensure high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.

Responsibilities include:
  1. Evaluating member needs, barriers to care, social determinants of health, and developing tailored care plans.
  2. Collaborating with providers to identify resources and address unmet needs.
  3. Identifying problems and barriers to care, providing appropriate interventions.
  4. Coordinating care between members, families, caregivers, and providers.
  5. Monitoring and updating care plans based on member progress.
  6. Providing resource support for various services based on assessments.
  7. Facilitating access to care and services through outreach.
  8. Documenting all activities to ensure compliance.
  9. Educating members and families on health conditions and care options.
  10. Providing feedback for quality improvement.
  11. Other duties as assigned.
Qualifications:
  • Degree from an accredited nursing school or a Bachelor's in Nursing with 2-4 years of experience.
  • RN license in Illinois or a compact state.

Pay Range: $55,100 - $99,000 annually.

Centene offers comprehensive benefits including health insurance, 401K, tuition reimbursement, paid time off, and flexible work options. Salary is commensurate with experience and other factors.
