Job summary

Centene Corp. is seeking a Remote Care Manager (RN) based in Ohio to assess, develop, and coordinate comprehensive care plans for members with complex needs. The role requires an RN license and Ohio residency with local travel for in-person visits when needed.

The position entails educating members and families, coordinating with providers, tracking progress, and adjusting plans to improve outcomes, all while ensuring compliance with regulations and company policies.

Qualifications

  • Requires degree from an accredited school of nursing or a Bachelor's degree in Nursing with 2–4 years of related experience.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
  • Authorized to work in the U.S. without visa sponsorship now or in the future.

Responsibilities

  • Evaluates member needs and barriers to care and facilitates a person-centered plan.
  • Develops ongoing care/service plans and coordinates with providers to address unmet needs.
  • Identifies problems/barriers to care and applies appropriate care management interventions.
  • Coordinates between member/family and care provider team to ensure appropriate, person-centered care.
  • Provides ongoing follow-up and monitoring, revising care plans as needed to meet goals.
  • Provides resource support for local services (employment, housing, independent living, etc.)
  • Facilitates care management with providers to ensure timely access to needed care
  • Performs telephonic, digital, home or site outreach to assess needs
  • Collects and maintains member information to ensure compliance with state/federal and payer requirements
  • Delivers education to members and families on disease processes, care options, and benefits
  • Provides feedback to leadership to improve care delivery and cost effectiveness
  • Performs other duties as assigned by leader to meet business needs
