Job summary

Centene is seeking a qualified RN Care Manager to develop and coordinate individualized care plans for members with complex physical needs, focusing on education and outcomes. This remote role emphasizes collaboration with providers to ensure timely access to services and adherence to care plans.

Required are an active New Jersey RN license, 2–4 years of relevant experience, and a degree from an accredited nursing program.

• State Licensure and/or Compact State Licensure required

Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop ongoing care/service plans and collaborate with providers to address unmet needs
• Identify problems/barriers to care and provide appropriate interventions
• Coordinate between members, families, and care providers for person-centered care
• Monitor member status and progress towards care plan goals, revising as necessary
• Provide resource support for local services based on assessments
• Facilitate care management and ensure timely access to needed services
• Perform outreach to assess member needs and collaborate with resources
• Document all member information and care management activities for compliance
• Educate members and families on disease processes and healthcare options
