Description

The Nurse Case Manager (NCM) is responsible to manage a caseload of patients across the continuum of healthcare services and assure optimal quality outcomes, patient satisfaction, and appropriate use of resources. The NCM is responsible for directing an organized, patient-centered interdisciplinary team system that coordinates the delivery of healthcare services by continually monitoring, evaluating, and modifying the treatment plan to achieve optimal patient outcomes and efficacious use of resources.

Required Qualifications:

  • Graduate of an accredited school of nursing.

  • Montana Registered Nurse License upon hire.

  • 3 years - Experience in Clinical Nursing.

  • 1 year - Hospital case management experience.

Preferred Qualifications:

  • Current Case Manager Certification upon hire.
