Title: Care Manager (RN)

Location: Remote SC
Duration: 04 Months (Potential to extend)

Job Description:

• Transition of Care Case Management for Medicaid members, telephonic outreaches to facilities and members. Assist facility staff with discharge planning efforts for members.

Performance Expectations/Metrics:

• Outreaches to members within 10 business days of hospital discharge.

First Day Activities:

• Setting up laptop and equipment, meeting with HR, prep work for 3-4 week Learning Journey.

Required Skills/Experience:

• 1. 2+ years of nursing experience.
• 2. Case management experience preferred, not required.
• 3. Great computer skills and knowledge of navigating systems.
• 4. Commitment.

Education Requirement:

• RN license (SC).

Preferred Education:
