Job Description: Monogram Health is seeking a skilled Registered Nurse eager to make a difference in patients' lives. The Care Manager RN is a key member of an integrated Care Team, including an Advanced Practice Provider and a Social Worker. The patients often face multiple serious diseases. Registered Nurses help improve quality of life at home and slow kidney disease progression, promoting positive health outcomes.

Roles And Responsibilities

• Collaborate with medical providers to develop and adapt care plans
• Conduct in-home care visits
• Monitor biometric data and follow protocols for interventions
• Manage medications and coordinate with pharmacists and prescribers
• Perform health assessments and surveys
• Educate patients on CKD, ESRD, dialysis, and comorbidities
• Encourage treatment adherence
• Engage family and support groups in care
• Serve as primary contact for patient questions
• Provide education on medications, conditions, diet, exercise, lifestyle
• Facilitate proactive care conversations, Advance Care Plans, ESRD options
• Measure vital signs and escalate concerns
• Handle enrollment and onboarding
• Support patients post-hospitalization
• Document updates in care management platform
• Coordinate with dialysis providers for seamless care transitions

Position Requirements

• Frequent local travel for in-home visits
• Basic Life Support (BLS) certification, company supports certification
• Occasional travel to Brentwood, TN for training
• Self-motivated with ability to work independently
• Empathy and relationship-building skills
• Graduate of an accredited Nursing school
• Current RN license in the state of the job location
• 2+ years in care management or with CKD/ESRD patients
• Ability to take remote calls during nights and weekends
• Excellent communication skills
• Familiarity with Microsoft Office and mobile/web applications
