Qualifications

Care Manager - Positive Choices (Addiction & Recovery)

Status: Full-time (37.5 hours/week)

Pay Range: $45,000 - $60,000 (paid hourly)

Work Location:

Remote except for when conducting client visits or participating in required in-person meetings or training sessions.

Job Summary:

Care Managers in Council on Aging’s Positive Choices program make a difference every day by delivering comprehensive care management services to Humana Medicare plan members experiencing substance use disorders (SUD), including alcohol and other drug (AOD) addictions. This role focuses on proactive outreach, crisis intervention, recovery support, and coordination of treatment services. The Care Manager uses evidence-based tools and motivational interviewing to empower members toward recovery and improved health outcomes.

Essential Functions:

  • Conduct initial outreach and in-home assessments using tools such as ANSA, AWARE, PHQ-2, PHQ-9, and SBIRT.
  • Develop a member-centered action plan with measurable goals for managing medical conditions and substance use.
  • Coordinate referrals to substance use treatment providers, including inpatient detox, outpatient programs, and recovery support groups.
  • Facilitate communication between primary care providers, behavioral health specialists, and substance use treatment providers.
  • Use motivational interviewing to engage members in self-care and recovery.
  • Address barriers to care such as transportation, housing, or access to treatment.
  • Document all member interactions and interventions in accordance with program protocols.


Required Education, Licensing & Certifications:

  • Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or a related field.
  • Ohio Licensed Social Worker (LSW) or Chemical Dependency Certification (COA will fund certification if not currently held).


Experience:

  • Minimum of 3 years of post-degree experience working with individuals with substance use, addiction, and behavioral health conditions.
  • Prior experience completing electronic documentation.
  • Prior case management experience and experience working in a program with strict and multiple contractual requirements is a plus.


Skills & Abilities:

  • Proficient in motivational interviewing and evidence-based behavioral health interventions.
  • Strong understanding of addiction, recovery, and behavioral health systems.
  • Knowledge of community resources in Southwestern Ohio.
  • Excellent communication, problem-solving, and crisis management skills.
  • Proficient in Microsoft Office and electronic documentation systems.
  • Valid driver’s license and reliable transportation for travel within Ohio counties.
  • Ability to travel within the state of Ohio for training.
