Job description

Fortcom Health Services is currently looking to hire Part-Time Care Manager for Bedford, Burlington, Lawrence, Billerica, North Andover, Leominster, Woburn, Dracut, Chelmsford, Methuen, Townsend, and Fitchburg areas (North Shore) of Massachusetts to perform monthly home visits to Members of our Adult Foster Care Program.

Qualifications

A bachelor’s degree and two years of clinical experience in the care of elders or people with disabilities.

A social worker license from the Massachusetts Board of Registration

Responsibilities

The responsibilities of the care manager include:

Conducting initial and ongoing psychosocial evaluation of a member’s appropriateness for AFC

Selecting, training, evaluating, and supervising AFC caregivers in conjunction with the registered nurse

Participating in the development, implementation, and ongoing review of the AFC plan of care

Conducting on-site visits with each member at the qualified setting

Reviewing AFC caregiver logs at a minimum of every 30 days

Reporting changes in the member’s condition to the member’s AFC registered nurse;

Participating in discharge and transition planning and implementation
