Qualifications

• Current Illinois RN license.

Responsibilities

• Provide dedicated care management for orthopedic patients, coordinating across providers, therapies, imaging, and labs.
• Identify barriers to care and develop individualized care plans with patients and families.
• Educate patients and caregivers on diagnoses, treatment plans, medications, procedures, recovery expectations, and follow-ups.
• Provide direct clinical nursing support including assessment, triage, and procedure assistance within scope.
• Document thoroughly in the electronic medical record and coordinate referrals and tests as needed.
• Collaborate with the care team to improve workflows and patient access.

Together, we create an environment where you can grow, contribute, and help improve the experience and outcomes for every patient we serve.**About the Role**As an RN Care Manager in Orthopedics at Quincy Medical Group, you will combine hands-on clinical nursing with care management to help patients successfully navigate their orthopedic care. Working closely with orthopedic providers, patients, families, and other members of the care team, you will coordinate services, identify barriers to care, provide patient education, and support patients throughout treatment and recovery.In addition to care management responsibilities, you will provide traditional clinic nursing support based on patient and department needs.
