Job summary

SCO Family of Services in New York is seeking a Care Manager to coordinate care for children, adults, and families with complex needs. The role focuses on aligning medical, behavioral health, and social supports within NYSDOH Health Home guidelines, including documentation and collaboration with partners.

The Care Manager will travel regionally across Suffolk/Nassau and NYC boroughs, work in a hybrid setting, and adhere to regulatory standards while promoting patient-centered care and outcomes.

Responsibilities

Job description

PURPOSE OF THE POSITION: The Care Managers service those who are enrolled in Medicaid, have Two or more Chorionic Conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI) or Complex Trauma; ages 0-21 (Childrens Program) & 18 to end of life (Adult Program). The Care Manager plays a critical role in achieving the overall goals for coordination, integration and partnership in the delivery of care to children, adults and families with complex needs.

REPORTS TO: Care Manager Supervisor

SUPERVISES: Not Applicable

SPECIFIC RESPONSIBILITIES

The Health Home Care Manager delivers all six NYS Department of Health (NYS DOH) Health Home Core Services, including:

  • Care Coordination and Health Promotion
  • Individual and Family Support
  • Referral to Community and Social Support Services
  • Use of Health Information Technology to Link Services

Additional responsibilities include:

  • Conducting NYSDOH-required assessments and developing compliant, person-centered Plans of Care
  • Coordinating medical, behavioral health, substance use, and social service supports
  • Engaging members through outreach, advocacy, and ongoing follow-up
  • Collaborating with hospitals, providers, Manage Care Organizations (MCO), and community partners
  • Maintaining accurate, timely documentation in NYSDOH-approved systems
  • Meeting Health Home productivity, engagement, and quality benchmarks
  • Participating in case conferences, audits, supervision, and training
  • RELATIONSHIP WITH OTHERS & IN THE WORKPLACE
  • Demonstrates professionalism, maintains appropriate boundaries, and follows the agency's

  • WORKING CONDITIONS

This is a hybrid office- and field-based position requiring regular travel throughout Suffolk and Nassau Counties and select boroughs of New York City. Travel may occur multiple times per week based on program needs.

The role includes scheduled in-office days, with meetings and trainings held in person at our Brooklyn or Dix Hills locations, as well as virtually as needed. While every effort is made to assign care managers to cases within their designated catchment area whenever possible, flexibility to travel for service delivery is required. Applicants must reside in New York State.

Care Manager must have one of the following:

  • A Masters with one year of relevant experience, OR
  • A Bachelors of Arts or Science with two years of relevant experience,
  • A License as a Registered Nurse with two years of relevant experience, OR

Relevant experience includes care coordination, case management, social services, behavioral health, medical care navigation, or community-based services.

All Health Homes Serving Children (HHSC) Care Managers must:

  • Complete Children Adolescent Needs Assessment (CANS-NY) training
  • Be CANS-NY certified annually
  • Care Managers: minimum 70%
  • Bilingual Preferred ( Spanish )
