Lawndale Christian Health Center is seeking a Care Manager-MSW to join a multidisciplinary team in Chicago. You will coordinate care plans for patients with high medical needs, collaborating with RNs, clinicians, pharmacy, and community agencies to ensure comprehensive, person-centered care.

The role requires a Master’s in Social Work, strong communication, and the ability to navigate multiple programs to support patients’ social and health needs.

Qualifications

• Masters Degree in Social Work from an accredited university.
• Bilingual in English and Spanish preferred.
• Two or more years of relevant experience preferred.
• Excellent communication with patients, staff, and community agencies.

Responsibilities

• Provide care management services for high-need patients as part of a care team.

Position Summary

The Care Manager-MSW is responsible for working as part of a care team to provide care management services for patients with high medical needs and patients of various risk groups. The Care Manager- MSW will coordinate with care team providers, RNs, other LCHC staff (including pharmacy, referral coordination and behavioral health), and community agencies regarding patient care/care management for patients in specifically defined patient populations in order to facilitate and implement a comprehensive, multi-disciplinary approach to manage health conditions, utilization of resources, patient self-care, and the implementation and evaluation of a care plan as it applies to non-clinical needs.
