Job summary

Altais Health Solutions in the San Francisco Bay Area seeks a Care Manager LVN to coordinate care for hospitalized members at risk, working with Health Management Operations and Health Value Teams to ensure appropriate admissions, levels of care, and timely discharges.

Qualifications

• Minimum of 3-5 years recent acute/hospital clinical experience.
• Computer skills proficient, experience in EMR, Microsoft Office Suite.
• Desired experience in managed care concepts and practice.

Responsibilities

• Conducting Initial and Concurrent Review and Discharge Planning/Care Coordination, utilizing evidence-based criteria and collaboration with Medical Directors to ensure appropriate admissions, level of care and timely discharges.
• Monitoring and continually assessing member severity of illness, intensity of services and response to treatment; make recommendations for continued stay or transition to lower level/tertiary care and coordinate member transitions.
• Apply clinical knowledge, critical thinking, and strong decision-making approaches.

As Care Manager LVN, you'll establish relationships and foster respect and rapport with our Health Management Operations and Health Value Teams and will work closely with our Provider Partners from the health care team responsible for the management and coordination for our hospitalized members at risk, and at their most vulnerable. You will focus on:

• Applying clinical knowledge, critical thinking skills, as well as your solutions-oriented problem-solving and strong decision-making approaches.

This position is a field-based opportunity in the San Francisco Bay area.

Skills, Experience & Education You Bring:

• Current unrestricted California LVN licensure required
• Minimum of 3-5 years recent acute/hospital clinical experience.
• Computer skills proficient, experience in EMR, Microsoft Office Suite.
• Desired experience in managed care concepts and practice.

Base Salary for this position is $41.00 - $52.00/hr
