Job description

LOCATION: Remote - must live in or near Rowan County, North Carolina. The position requires travel and the candidate must reside in North Carolina or within 40 miles of the state border.

GENERAL STATEMENT OF JOB

The Care Manager Licensed Professional ("Care Manager - LP") provides proactive care coordination and intervention for eligible Vaya Health members. This role involves assessing members' needs, developing care plans, coordinating services across various health domains, and supporting transitions from facilities to community settings. The position is mobile, working in members' communities and collaborating with other staff, providers, and community stakeholders. Clinical skills are essential for assessment, care planning, and providing recommendations.

Key responsibilities include:

  1. Using Vaya's Care Management software and maintaining accurate documentation.
  2. Conducting health risk assessments and comprehensive bio-psycho-social evaluations.
  3. Developing and updating person-centered care plans.
  4. Engaging with members and care teams to ensure appropriate services and supports.
  5. Supporting transitional and diversion care efforts.
  6. Participating in multidisciplinary team meetings and community collaborations.
  7. Ensuring compliance with HIPAA and other regulations.
  8. Maintaining professional licensure and completing required training.

Qualifications & Education

Master's degree in a relevant human services field is required, with specific experience depending on the degree. Valid licensure (e.g., RN, LCSW, LCMHC, etc.) is mandatory. Experience with mental health, LTSS, I/DD, or TBI populations is preferred.

Physical Requirements

Includes visual acuity, mobility, and the ability to drive and sit for extended periods.

Residency Requirement: Must reside in North Carolina or within 40 miles of the border.

Salary: Commensurate with qualifications and experience. The position is exempt, with no overtime pay.
