Job summary

4th Second is seeking a Care Manager to provide comprehensive case management for individuals transitioning from the justice system and those experiencing homelessness in Vallejo, CA. You will help clients set and achieve goals toward self-sufficiency, with home visits, outreach, and transportation support as needed.

The role spans Levels I–III with opportunities for professional development; a hybrid in-office/community model is offered, with 1 remote day after 90 days.

Qualifications

  • Bachelor's degree in social services, psychology, or related field and/or equivalent experience.
  • Extensive experience with low-income and/or homeless populations and advanced case management.
  • Proficient with Google Docs, MS Office, and EHR platforms.
  • Advanced typing skills and reliable transportation with DMV-acceptable record.
  • Successful background checks including Live Scan.

Responsibilities

  • Carry a caseload of 18-22 participants with regular contact at least monthly.
  • Conduct initial assessments to identify clients' needs, strengths, and barriers.
  • Develop and implement personalized case management plans for each client.
  • Provide ongoing support, counseling, and advocacy to clients for access to resources.
  • Coordinate with community orgs to facilitate housing, employment, education, healthcare, and supports.
  • Maintain up-to-date client records, case notes, and documentation.
  • Participate in team meetings, case conferences, and trainings to improve practice.

About the Role

As a Care Manager you will be responsible for providing comprehensive case management services to clients. In this dynamic role you will work closely with individuals who are transitioning from the justice system as well as individuals experiencing homelessness by helping them navigate various challenges and barriers they may face. This position assists and aids program participants with effectively setting, managing and attaining their goals to become self‑sufficient and financially independent whether through employment or assistance with social services. The Care Manager may perform home and encampment visits, engage in outreach services with and without community partners, provide transportation for clients, work a flexible schedule, and must be willing to accommodate participants who do not have reliable transportation.

Level I-III

We offer Care Manager positions ranging from Levels I-III based on experience and to promote professional development. A role as a Care Manager Level II & III requires advanced skills and experience in managing complex cases, high‑level documentation, and providing expert guidance and support to participants.

Monday-Friday 9 am - 4:30 pm; 35 per week (7 hour workday); Occasional weekends and evenings are required

Location: Vallejo, CA - in office & community; 1 remote day may be granted after 90 day introductory period

What You Will Do
  • Carry a caseload of 18-22 participants maintaining regular contact (at least monthly, based on acuity level) with program participants. Conduct outreach to meet clients where they are including curbside communities, shelters, and other places people may be living
  • Conduct initial assessments to identify clients' needs, strengths, and barriers
  • Develop and implement personalized case management plans for each client
  • Provide ongoing support, counseling, and advocacy to clients, ensuring they have access to necessary resources and services
  • Collaborate with community organizations, social service agencies, and healthcare providers to coordinate and facilitate access to housing, employment, education, healthcare, and other support services
  • Monitor clients' progress and provide regular updates to the program director and other team members
  • Maintain accurate and up-to-date client records, case notes, and documentation
  • Participate in team meetings, case conferences, and training sessions to enhance professional development and share best practices
  • Commit yourself to treating each community member with respect and dignity
  • Maintain database records, including client intakes, program outcomes
  • Adhere to laws regarding confidentiality and reporting requirements; maintain knowledge of HIPAA certification standards
Minimum Qualifications
  • Bachelor’s degree in social services, psychology, or a related field, with relevant certification from a four‑year program and/ or equivalent experience
  • Extensive experience working with low‑income and/or homeless populations, demonstrating advanced clinical skills and case management expertise
  • Proficient computer skills, including Google Docs, Microsoft Office Suite, and EHR platforms
  • Advanced typing skills
  • Experience working in diverse settings with people across all socio‑economic spectrums and a wide variety of personalities
  • Access to reliable personal transportation required, including a DMV record that permits the driver to be insured under 4th Second’s automobile coverage
  • Pass Required Background Checks: Successfully complete Live Scan and other background checks as a prerequisite for hiring, ensuring compliance with safety and security protocols set forth by 4th Second
Preferred Qualifications
  • Master’s degree with an emphasis in social services, psychology, or related fields
  • Extensive experience working with low‑income and/or homeless populations
  • Bilingual in English and Spanish
Physical Qualifications
  • Engage in verbal and written communication
  • Operate a computer, smartphone, and other office tools and equipment
  • Reach hands up above one's own head
  • Stand and sit for at least 1 hour at a time
  • Able to walk moderate distances, such as for a duration of 15 minutes and up and down moderate gradients (i.e., hills, stairs)
  • Crouch, kneel, or squat for at least 5 minutes at a time
  • Able to traverse uneven ground (i.e., walk along terrain where the ground is sloped and rocky)
  • Able to carry and/or lift up to 25 lbs
  • Visually or otherwise focus and navigate in immediate surroundings
  • Reasonable accommodation will be provided in accordance with applicable laws and policies
  • Operate a moving vehicle, including some night driving
Work Environment

This position operates in both residential care settings and the community. The role may involve exposure to unsanitary conditions, challenging behaviors, and various environmental factors. Staff are expected to follow all safety protocols and demonstrate cultural competency and trauma‑informed care in all interactions.
