As a Care Manager you will be responsible for providing comprehensive case management services to clients. In this dynamic role you will work closely with individuals who are transitioning from the justice system as well as individuals experiencing homelessness by helping them navigate various challenges and barriers they may face. This position assists and aids program participants with effectively setting, managing and attaining their goals to become self‑sufficient and financially independent whether through employment or assistance with social services. The Care Manager may perform home and encampment visits, engage in outreach services with and without community partners, provide transportation for clients, work a flexible schedule, and must be willing to accommodate participants who do not have reliable transportation.
We offer Care Manager positions ranging from Levels I-III based on experience and to promote professional development. A role as a Care Manager Level II & III requires advanced skills and experience in managing complex cases, high‑level documentation, and providing expert guidance and support to participants.

Monday-Friday 9 am - 4:30 pm; 35 per week (7 hour workday); Occasional weekends and evenings are required
Location: Vallejo, CA - in office & community; 1 remote day may be granted after 90 day introductory period
This position operates in both residential care settings and the community. The role may involve exposure to unsanitary conditions, challenging behaviors, and various environmental factors. Staff are expected to follow all safety protocols and demonstrate cultural competency and trauma‑informed care in all interactions.
