Overview

The path to healthcare and improving your life is complicated and challenging and even more so for our vulnerable community members who experience homelessness, severe mental illness, substance use disorder, and/or serious medical challenges. As an Integrated Care Manager you provide support to these highly vulnerable participants through intensive coordination of health and health-related services. You will coordinate care and services to help meet the physical, behavioral, dental, developmental, and social needs of the people you serve. These services improve access to care for people with complex needs and who are facing difficult life and health circumstances. You will develop a comprehensive care plan with each participant and act as their primary point of contact to navigate the various system of care they need to access to meet their needs. Additionally, you will ensure all providers involved in the participant’s care are communicating with each other and working together. Ideal candidates for this position are outgoing and creative, with knowledge of the local resources available to support the people we serve.

Essential Duties and Responsibilities
  • Conduct outreach and engagement efforts to connect potential participants to Enhanced Care Management (ECM) benefit.
  • Assess and identify needs of each ECM participant to draft an individualized care plan with them.
  • Works with clients in group and individual settings to discuss, break down, and resolve issues/barriers interfering with the plan of care.
  • Refer and link clients to appropriate medical and mental health care, aiding them with scheduling and making it to appointments.
  • Acts as an advocate, helping people communicate their needs to their care providers. Facilitates resolution of complex care challenges in a multi-disciplinary forum while concurrently advocating for the System of Care (i.e., keeping client engaged with care even when it is challenging to access).
  • Takes and records client vital signs, including blood pressure. Completes regular screenings of vital signs, wellness status, and health needs.
  • Documents each service provided to clients in a timely manner.
  • Communicate and collaborate with partnering service providers regarding treatment, needs, progress, and care of clients, as allowed by regulations permitting the sharing of information between providers to benefit their client’s accessing healthcare, housing, and services.
  • Develops and maintains strong knowledge of local resources. Seeks out additional resources as needed, remains up to date on changes in available resources and eligibility criteria.
Qualifications Classifications and Qualifications
  • Care Manager I: An individual who has less than two (2) years of FTE experience in mental health related field providing direct mental health service.
  • Care Manager II: An individual who has at least two (2) years but less than four (4) years of FTE experience in a mental health related field providing direct mental health services.
  • Care Manager III: An individual who has at least four (4) years of full-time/equivalent (FTE) experience in a mental health-related field providing direct mental health services. ** Up to two (2) years of the four (4) years of experience can be substituted by years of education in a mental health-related field. There is a minimum requirement of two (2) years of actual work experience.

* *The education requirement for a Care Manager III - must be a minimum of two (2) years of education (60 semester or 90 quarter units) with a minimum of 12 semesters (18 quarter) units in a mental health-related subject area such as child development, social work, human behavior, rehabilitation, psychology, or alcohol and drug counseling.

  • Minimum of one (1) year of experience working in a social work-related position providing direct client services. Prefer candidates with experience working with people experiencing homelessness.
  • Strongly desired – Candidates with lived/personal experience with homelessness, mental illness, substance use disorder, and/or other challenges experienced by the people served.
  • Must have basic knowledge of Microsoft Suite (Excel, PowerPoint, Outlook, etc.).
  • Must be able to provide written verification of experience and education upon hire.
  • Must have ability to occasionally or regularly work a flexible schedule, including weekends and evenings.
  • Strongly desired – Candidates that speak and/or are fluent in Spanish or American Sign Language (ASL).
  • Preferred – Training in harm reduction, Housing First, trauma-informed, low barrier service models.
Conditions of Employment
  • May work across all agency programs as needed.
  • Occasional flexible work schedule required to meet individual client needs.
  • Must possess a valid CA Driver’s License, current auto insurance, and a clean DMV printout (no more than 3 points).
  • Must pass a clear background check with CACI, DOJ & FBI.
  • CPR and First Aid Certified.
  • TB Exam and Physical at the time of employment.
Driving Requirement
  • Driving is a required function of this position. Employees must use a personal vehicle that meet all agency requirements, including maintaining liability insurance that meets or exceeds state requirements.
  • Employees are responsible for ensuring the timely renewal of their vehicle insurance policies and providing updated documentation to the agency to remain in good standing and compliant with position requirements.

  • Comprehensive onboarding including CPR/First Aid, Crisis Communication/Pro-ACT, Trauma-Informed Care, Motivational Interviewing, and Collaborative Problem Solving.
