Job Summary

Molina Healthcare Services (HCS) works with members, providers and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care across the continuum, including behavioral health and long term care, for members with high need potential. HCS staff work to ensure that patients progress toward desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service.

This position will support our Washington State Plan. We are seeking a candidate with an LCSW, LMSW or LMFT licensure. Candidates should have experience with motivational interviewing and working with members with co-morbid conditions. Experience with Behavioral Health conditions is highly preferred. Work schedule: Monday-Friday, 8:00 AM to 5:00 PM PST

Remote position with field travel within Clark, Cowlitz, Multnomah counties and/or within 25 miles of Vancouver, WA.

WA LCSW, LMSW or LMFT licensure required.

Knowledge/Skills/Abilities
  • Complete clinical assessments of members per regulated timelines, determining eligibility for case management based on clinical judgment, health status, psychosocial wellness, and assessment triggers.
  • Develop and implement case management plans collaboratively with members, caregivers, healthcare providers, and support networks to address needs and goals.
  • Conduct telephonic, face-to-face, or home visits as required.
  • Monitor care plans regularly, evaluate effectiveness, document interventions, and suggest adjustments.
  • Maintain ongoing member caseload for outreach and management.
  • Promote service integration, including behavioral health and long-term services, to enhance continuity of care.
  • Implement specific Molina wellness programs, such as asthma and depression management.
  • Facilitate interdisciplinary care team meetings and collaborate informally with care teams.
  • Use motivational interviewing and Molina clinical guideposts to educate, support, and motivate members.
  • Assess barriers to care, provide coordination, and address member concerns.
  • Collaborate with RN case managers/supervisors as needed.
  • Behavioral health and social science case managers may provide consultation, resources, and recommendations.
  • Local travel of up to 40% may be required depending on member needs and regulations.
Job Qualifications

Required Education: Any of the following:

  • Completion of an accredited LVN or LPN program, or
  • Bachelor's or Master's degree in social science, psychology, gerontology, public health, social work, or related field.

Required Experience: 1-3 years in case management, disease management, managed care, or behavioral/medical health settings.

Required License: Active, unrestricted license in good standing. Valid driver's license with a good record and reliable transportation.

State-Specific Requirements: For roles serving Wisconsin's Family Care programs, a Bachelor's Degree and at least one year of professional experience are required.

Preferred Experience: 3-5 years in relevant fields.

Preferred Licenses/Certifications: LCSW, APSW, CCM, CHEP, LPC/LPCC, Respiratory Therapist, LMFT, or similar.
