Job summary

Cheer Home Care in San Diego is seeking a dedicated Care Case Manager to design and oversee individualized care plans for seniors in the community.

You will collaborate with office staff and caregivers, ensure clear communication with clients and families, adapt plans as needs evolve, monitor progress, document outcomes, and advocate for residents' wellbeing.

Qualifications

• MSW, Nursing, or related field required or preferred.
• Strong communication and organizational skills required.
• Pass Live Scan fingerprint clearance (LIC 9163) is listed in requirements.
• Valid driver’s license and proof of insurance required.
• Cell phone with Internet access.
• Proof of negative TB results.
• Willingness to work one weekend a month as back-up on call.

Responsibilities

• Create and manage care plans for clients.
• Coordinate with office team and caregivers to ensure optimal care.
• Provide feedback to clients, client representatives, and caregivers.
• Maintain thorough documentation and comply with relevant regulations.
