About the Organization Thrive Alliance helps individuals access resources and support services that improve their lives and builds stronger communities. As the "umbrella" over several programs, we provides services that help the elderly, disabled, and low-income individuals in 5 counties of south central Indiana: Bartholomew, Brown, Decatur, Jackson, and Jennings. These programs include:

Aging and In-Home Services - as the local Area Agency on Aging, Area 11, Thrive Alliance provides Care management and resources to assist those over the age of 60 to remain at home or in a community setting.

Aging and Disability Resource Center (ADRC): a highly visible and trusted place where people can turn for information on a full range of support options.

Adult Guardian Services: Provides guardianship services for disabled adults in need of support and legal assistance.
Nutrition Services: Provides congregate meal site where seniors can come have a hot meal and interact with others in a comfortable environment. Educational program often offered. Current meals sites include Nashville, IN, Seymour, IN, Crothersville, IN, North Vernon, IN and Columbus, IN.
First Steps: Provides case management, assessment, advocacy, and resources to assist families with a child under age 3 diagnosed with a developmental disability
Caregiver Support: Provides support and education to families and caregivers of individuals dealing with dementia
Housing Partnerships: Provides safe and affrodable housing to low-income, seniors and disabled individuals

Description

Thrive Alliance is seeking a full-time Care Manager.

Care Managers are responsible for coordinating services that help keep individuals with disabilities and older adults in their homes. This role focuses on assisting with care coordination, ensuring effective communication between clients, families, and healthcare providers.

Key Outcomes, Expectations and Responsibilities:

Conduct reassessments of individual's needs as circumstances change and as required by program standards

Periodic updates of care plans to include enhancement or termination of services based on need

Manage the quality home-based services provided to the individual as needed and no less than the frequency required by program standards

Identify and track community needs and gaps in services

Documentation of all direct and non-direct case management related activities within required timeframe

Submission of all reportable instances to the State and Protective agencies as required

Maintain strict confidentiality and compliance with HIPPA guidelines

Provides peer to peer training for new hires Other duties as assigned

Must possess knowledge of contractual health insurance or risk arrangements.

Proficient with Microsoft Office Suite,Teams and associated software to meet department requirements.

Position Requirements

The minimum of a Bachelor's Degree from an accredited institution, in a Human Services field such as Psychology, Social Work, Education, Nursing or related.

The ability to drive for the agency: a valid driver's license and current auto insurance

Successful individuals are empathetic, friendly, and professional, have good computer and time management skills, and are self-motivated and able to work with minimal supervision.
