Qualifikationen

• Experience using electronic health record, scheduling, care management, or customer relationship management systems.
• Strong problem-solving, documentation, and follow-through skills.
• Ability to interpret established procedures and communicate clearly with patients, families, and care teams.

Job Description Summary

We are excited to announce an opening for a Care Manager in our Lakeland, Florida location. A Care Manager supports patients and families through routine and moderately complex navigation needs by coordinating appointments, care team communication, community resources, and follow-up activities. Resolves common access barriers, interprets standard workflows, and helps patients understand next steps across care settings, payer requirements, and internal service channels.

The Care Manager carries out defined clinical support processes under moderate supervision, applying working knowledge to tasks that occasionally require interpretation or deviation from standard procedures. Decisions are procedure-driven and impact day-to-day patient interactions and team workflow. This job handles routine and moderately non-routine situations, communicating clearly with patients, members, and clinical staff.

Compensation Range

$20.90 to $31.35

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
