Care Manager II (Hybrid, North Carolina Based)

Join to apply for the Care Manager II (Hybrid, North Carolina Based) role at Alliance (now part of Merrick & Company).

This position leads communication among care team members and serves as the primary contact for members. The Care Manager conducts comprehensive assessments and develops care plans for Tailored Plan recipients, facilitating communication among providers.

This is a full-time hybrid role. The candidate must be available to report onsite at the Alliance Home Office in Morrisville, NC, if needed, and travel weekly within Mecklenburg County for home visits, provider meetings, and community engagements.

Responsibilities & Duties
  1. Assessment & Planning: Complete assessments, develop care plans, assign interventions, submit referrals, evaluate service appropriateness, and collaborate with the care team to develop accurate, person-centered plans.
  2. Support & Monitoring: Initiate contact with members, verify information, provide education, recognize crises, coordinate care transitions, and ensure ongoing service adherence through monitoring and communication.
  3. Documentation: Obtain and upload documentation, open new episodes, record activities, escalate complex cases, share information with stakeholders, and ensure compliance with documentation standards.
Minimum Education & Experience

Master's degree in Human Services or related field with at least two years of relevant experience and licensure in North Carolina (LCSW, LMFT, LCAS, LCMHC, LPA), OR a licensed Registered Nurse with two years of relevant experience.

  • Person-centered planning, assessment skills, knowledge of mental health diagnoses, Medicaid benefits, motivational interviewing, proficiency in Microsoft Office, strong communication, diplomacy, and decision-making skills.

Salary Range

$66,240 - $86,112 annually, based on experience and qualifications. Benefits include health insurance, retirement plan, flexible schedules, paid time off, and dress code flexibility.
