This position leads communication among care team members and serves as the primary contact for members. The Care Manager conducts comprehensive assessments and develops care plans for Tailored Plan recipients, facilitating communication among providers.
This is a full-time hybrid role. The candidate must be available to report onsite at the Alliance Home Office in Morrisville, NC, if needed, and travel weekly within Mecklenburg County for home visits, provider meetings, and community engagements.
Master's degree in Human Services or related field with at least two years of relevant experience and licensure in North Carolina (LCSW, LMFT, LCAS, LCMHC, LPA), OR a licensed Registered Nurse with two years of relevant experience.
