Responsibilities:
- Conduct comprehensive assessments and ongoing re-assessments of clients, including psychosocial, physical, mental health, environmental, and spiritual needs.
- Write comprehensive assessments and develop, initiate, and review the Community Living Plan in collaboration with the client, ensuring it is client-centered and compliant with program guidelines.
- Conduct home visits, hospital, and nursing facility visits; escort clients to medical appointments as needed.
- Identify, arrange, and monitor appropriate community services, understanding Medicare, Medi-Cal, and other entitlement programs.
- Establish and maintain care management relationships, providing respect, dignity, support, crisis intervention, advocacy, and therapeutic interventions.
- Meet with clients regularly, review and modify their Community Living Plan as needed.
- Document all case management activities within 24-48 hours, updating the Community Living Plan accordingly.
- Maintain proper paperwork and a clear charting system for continuity of care.
- Evaluate client Purchase of Service needs and follow up to ensure timely service provision.
- Educate clients and support networks about available resources.
- Communicate effectively with community providers, maintaining confidentiality.
- Monitor the quality and quantity of services provided by other providers.
- Collaborate with the team to evaluate clients’ ability to remain safely at home and coordinate placements as needed.
- Participate in research, data collection, and quality improvement initiatives.
- Attend team meetings, contribute to communication, and utilize supervision effectively.
- Incorporate NASW ethical and legal standards into interactions.
- Understand and apply regulatory and procedural requirements of IOA.
- Participate in continuing education to enhance relevant skills and knowledge.
- Perform other related responsibilities as assigned.
Education:
- M.S.W. or equivalent Masters degree (e.g., MPH, MFT) with relevant experience, or
- B.A./B.S. with at least two years of relevant social work experience, capable of autonomous assessment and intervention.
Background and Experience:
- At least one year working with disabled or older adults.
- Understanding of medical and psychosocial issues of impaired adults.
- Experience with mental health and substance abuse preferred.
- Strong communication skills with clients and support networks.
- Case management experience in community healthcare.
- Detail-oriented with problem-solving skills and ability to prioritize.
- Computer literacy required.
Compensation:
Range: $84,077 - $100,485/year
