Job summary

Monarch Healthcare in North Carolina seeks a Care Manager to plan, coordinate, track, and monitor care for individuals to achieve BH I/DD Tailored Plan goals. You will engage with clients, families, and supports to drive person-centered care and budget planning.

The role requires RN licensure or a bachelor’s degree in a related field, at least 2 years of I/DD/TBI experience, and NC qualification per 10A-NCAC. A Spanish-speaking candidate is preferred.

Qualifications

  • RN licensure OR bachelor’s degree in a health-related field.
  • Two years of direct I/DD or TBI experience.
  • Meets North Carolina Qualified Professional definition per 10A-NCAC 27G.0104.
  • Spanish-speaking is preferred.

Responsibilities

  • Actively engages with individuals to assess, coordinate, health promotion, and transition care up to ISP.
  • Adheres to deadlines and agency policies/procedures.
  • Assists individuals in planning, emphasizing the person’s choice.
  • Coordinates care team meetings with the individual, family, and supports.
  • Refers to community resources and supports individuals’ healthcare needs.
  • Reviews data to monitor utilization, compliance, and outcomes.
  • Travel may be required and driving may be involved.

Meet North Carolina’s definition of a Qualified Professional per 10A-NCAC 27G .0104Spanish-speaking preferredJob Description Summary:The primary responsibilities of the care manager is to plan, coordinate, track, and monitor care to individuals to achieve the key goals of the BH I/DD Tailored Plan.What You'll Do:• Actively engages with individuals receiving care management services through assessment, care coordination, health promotion, and comprehensive transitional care up to inform the ISP.• Adheres to all deadlines as outlined by the service definition. Demonstrate knowledge of and comply with all agency policies/procedures and service definition.• Assists the individual supported to direct the planning process/plan development, to the extent desired by the individual, with emphasis on the person’s choice. Provide education regarding services to all parties involved in the care and support of the individual.• Coordinates care team meetings with the individual, family, and paid or unpaid supports identified in the person’s life.• Coordinates appropriate referrals to community resources and empowers individuals to be responsible for their own healthcare and personal needs. Referrals will include, but are not limited to, linking with supports for behavioral health, physical health, and Social 6. Determinants of Health (SDOH) needs.• Manages transitions of care for people transitioning from one clinical setting to another and/or life transitions as outlined in the service definition.• Communicates effectively with individuals utilizing person-centered practices such as motivational interviewing, person-centered thinking, and trauma informed care. Communicates effectively with external stakeholders such as providers, MCOs, other natural support as needed.• Educates individuals/families on methodology for budget development and mechanisms available to modify the individual budget.• Establishes collaborative relationships with community stakeholders within the assigned geographical region to improve resource linkage.• Identifies and provides emergency crisis response as necessary and following agency policies related to crisis.
