Job Description
The Care Manager I (CMI) is responsible for assessing resident psychosocial needs and coordinating support services for low-income older adults. The CMI plans and monitors services and interventions to ensure quality care.
Responsibilities
- Conduct comprehensive assessments and ongoing re-assessments of residents' psychosocial, physical, mental health, environmental, and spiritual needs.
- Write detailed assessments and develop client-centered Community Living Plans based on assessment information.
- Identify, arrange, and monitor appropriate community services, with knowledge of Medicare, Medi-Cal, and other entitlement programs.
- Establish and maintain care management relationships with clients and their significant others, offering respect, dignity, and support. Provide crisis intervention, advocacy, problem-solving, and therapeutic interventions.
- Meet with clients at least monthly, reviewing and updating their Community Living Plans as needed.
- Document all case management activities via progress notes within 24 hours, updating service plans accordingly.
- Maintain required paperwork and follow a clear, concise charting system for continuity of care.
- Educate and train clients and significant others about resources and self-management when possible.
- Maintain open communication with community providers, including physicians and social workers, ensuring confidentiality.
- Monitor the quality and quantity of services provided by other providers.
- Collaborate with clients, caregivers, and involved services to appropriately terminate clients when necessary, documenting the process.
- Participate in research and data collection as required.
- Promote ongoing Continuous Quality Improvement efforts.
- Attend and actively participate in team and program meetings, contributing to communication and problem-solving.
- Use supervision effectively, keeping open communication and providing caseload updates.
- Incorporate the ethical standards of the National Association of Social Workers into all interactions.
- Understand and apply regulatory and procedural requirements of Community Living Services and IOA policies.
- Attend continuing education and in-service training to enhance skills and knowledge relevant to the role.
- Perform other related responsibilities as assigned.
Qualifications
- B.A. or B.S. in Social Work or related field required.
- At least one year of experience working with disabled adults and/or frail elderly required.
- Knowledge of medical and psychosocial issues affecting functionally impaired adults and the elderly.
- Bilingual skills highly preferred, especially English plus Cantonese, Mandarin, and/or Spanish.
- Strong communication and presentation skills for working with the elderly, support systems, and healthcare teams.
- Case management experience in community healthcare preferred.
- Detail-oriented with good problem-solving and multitasking skills.
- Computer literacy required.
Physical & Language Requirements
- Ability to lift up to 25 lbs.
- Ability to reach, bend, walk.
- Five-finger manipulation.
- Clear speaking and writing skills in English.
Compensation
- Range: $71,797 - $85,808 annually.
This amount may vary based on experience, education, and other factors. It does not include additional benefits or non-monetary compensation.
