Job summary

SPARC Services and Programs is seeking a Care Manager to provide whole-person integrated care management to members on tailored plans in Davidson and Randolph Counties. You will guide individuals to navigate care, coordinate resources, and support overall well-being.

Qualifications

• Bachelor's degree in a Human Services field (psychology, sociology, another relevant human services area, or licensure as a registered nurse)
• Minimum 2 years of experience working directly with individuals with mental health, substance use, or other behavioral health conditions
• Valid driver's license and reliable transportation

Responsibilities

• Comprehensive assessments and individualized care plans
• Care team development and ongoing care coordination
• Transitional care management
• Health promotion and medication monitoring
• Addressing unmet health-related resource needs
• Promoting a system of care approach
• 24/7 on-call crisis support (rotational coverage)
