Trinity Health IHA Medical Group seeks a compassionate Care Manager – Oncology to join our outpatient care team. You will guide patients through chemotherapy with clinical triage, education, and coordination to improve outcomes and the patient experience.

This role offers a hybrid opportunity (up to 2 days/week after orientation) with a day shift focus and collaboration across providers, pharmacists, and behavioral health specialists to support transitions of care.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in the State of Michigan. CPR certification.
• CCM/CCP certification preferred.
• 3–5 years of oncology clinical experience in ambulatory/outpatient settings within the last five years.

Responsabilités

• Provide clinical guidance, patient education, and care coordination.

What We're Looking For:

• EDUCATION: Graduate of an accredited Registered Nurse program.
• CREDENTIALS/LICENSURE: Valid, unrestricted RN license in the State of Michigan. Valid CPR certification. Prefer CCM/CCP certification.
• MINIMUM EXPERIENCE: 3-5 years of clinical experience with oncology patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital, within the past five years: knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education.
