Job description

Join to apply for the Care Manager, Fluent in Bengali - 100% Remote role at Healthfirst.

The Care Manager plans and manages behavioral and/or physical care with members, working collaboratively with them, their supports, providers, and healthcare team members. Responsibilities include applying care management principles, engaging members, coordinating healthcare services, and improving health outcomes. The role is assigned to a specific product line such as CompleteCare, SNP, Medicaid/Medicare, PHSP, HARP, etc.

Duties and Responsibilities
  1. Advocate, inform, and educate beneficiaries on services, self-management techniques, and health benefits.
  2. Conduct assessments to identify barriers and opportunities for intervention.
  3. Develop care plans aligned with physicians’ treatment plans and recommend interventions.
  4. Generate referrals to providers, community resources, and other services to support health outcomes.
  5. Liaise between service providers to ensure coordinated care and address needs.
  6. Coordinate with the healthcare team to ensure care plan goals are person-centered and maximize outcomes.
  7. Identify opportunities for alternative care options based on assessments.
  8. Evaluate service authorizations for alignment with care and treatment plans.
  9. Contribute to corporate goals through ongoing care plan execution and member goal achievement.
  10. Document encounters and update care plans as needed, following documentation procedures.
  11. Occasional overtime and additional duties as assigned.
Minimum Qualifications
  • For Medical Care Management: NYS RN or LCSW/LMSW (any state); Pediatric experience preferred for PEDS roles.
  • For Behavioral Health Care Management: NYS RN or licensed psychologist, LCSW, LMSW, LMFT, LMHC, LPC (any state); 3 years experience in mental/behavioral health or addictions.
Preferred Qualifications
  • Strong interpersonal and assessment skills, fluency in Bengali, experience with community health practices, and managing member information.
  • Experience managing large caseloads and working in fast-paced environments.
  • Proficiency with Microsoft Office and electronic documentation systems.
