ECU Health is seeking a Care Manager responsible for coordinating patient care through collaboration with healthcare teams. This role focuses on enhancing care quality while managing costs effectively. The ideal candidate will hold an Associate Degree in Nursing or a Bachelor's in Social Work, with 0-3 years of experience in Medical Case Management.
This position operates on a 0.03 FTE basis with 8-hour shifts during the day. The starting pay is competitive based on local market standards, ranging from $31.50 to $45.91.
The Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family. The Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Care Manager helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements and patient satisfaction). The Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well‑organized and safe plan of care is established for every patient.
$31.50 - $45.91. Starting pay in the Outer Banks Hospital/Outer Banks Medical Group market may be higher to reflect local market competitiveness.
