Job summary

ECU Health is seeking a Care Manager to coordinate and optimize the plan of care across the care continuum. You will work with providers, the interdisciplinary team, patients and families to ensure high-quality outcomes and satisfaction while managing resources.

Responsibilities include psychosocial assessments, timely communication, and leading care plan development. This role requires 0–3 years in medical case management and relevant nursing or social work education, with a Monday–Friday

Qualifications

  • Education requirement includes nursing or social work background with relevant licensure where applicable.
  • Experience in medical case management or care coordination preferred.
  • Ability to collaborate with interdisciplinary teams and families.

  • Perform psychosocial assessments within departmental procedures and timeframes.
  • Communicate with providers and care team to support timely patient management.
  • Lead interdisciplinary team in plan of care development using clinical judgment.
  • Identify barriers to care and resolve system issues affecting diagnostics or treatment.
  • Arrange support services to improve outcomes with safe, sustainable plans.
  • Assess patient health behaviors and cultural factors to address barriers.

Job description

Position Summary

The Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family.

The Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Care Manager helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements and patient satisfaction).

The Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well-organized and safe plan of care is established for every patient.

Responsibilities
  • Manage, coordinate, and facilitate patient care progression throughout the continuum.
  • Completes psychosocial assessments using departmental procedures and within the required timeframes.
  • Works collaboratively and maintains active communication with providers, nursing and other members of the health care team to effect timely, appropriate patient management.
  • Utilizes critical thinking, assessment findings and clinical skills to lead interdisciplinary team in plan of care development and implementation.
  • Identifies delays or alterations in the plan of care and addresses/resolves system (internal and external) problems impeding diagnostic or treatment process to ensure delivery of care in the most appropriate setting.
  • Makes the necessary arrangements to facilitate and coordinate support services to significantly improve outcomes, through safe, sustainable plans of care and appropriate follow up.
  • Identifies and applies information about patient\'s health behaviors, cultural influences, and belief or value system. Identifies, addresses, and re-assesses based upon potential barriers to plan and works with healthcare team to address.
Minimum Requirements

Education: Associate Degree in Nursing or higher or Bachelors of Science in Social Work or higher is required.

If a Registered Nurse, current licensure as a Registered Nurse by the NC Board of Nursing is required.

Experience: 0-3 years of Medical Case Management

Pay Range

$65520.00 - $95501.12

Monday-Friday 8:00-16:30
