Overview

Care Manager - Bilingual English/Spanish (Healthcare)

The Care Manager works collaboratively with individuals with intellectual/developmental disabilities, their families, OPWDD, and relevant providers to ensure that all individuals supported are able to live safe, happy, and fulfilling lives. This includes identifying goals, building plans to reach those goals, and assisting with applying to a variety of supports and services to help the individual meet person-centered outcomes. Tri-County Care supports members of all ages and may include advocating for individuals with their schools, helping them find medical providers, and assisting with applications to Social Security, SNAP benefits, Waiver programs, and related supports.

Responsibilities
  • Collaborate with individuals, families, OPWDD, and providers to identify goals and develop person-centered plans.
  • Assist with applying to supports and services to meet individual outcomes.
  • Advocate for individuals with schools and coordinate access to medical providers.
  • Assist with applications to Social Security, SNAP benefits, Waiver programs, and related supports.
Location and Travel

This position is remote but requires periodic in-person visits. Candidates will need to be able to travel to conduct these visits via car or public transportation. Caseload will be located in or near the listed county but is not limited to the county listed.

This position requires applicants outside of New York City to have a valid driver's license and reliable transportation. Travel reimbursement is provided.

Work Environment

This position requires a quiet, distraction-free environment for working, or the ability to work from one of our regional offices.
