Essential Duties and Responsibilities

1. The role of a Care Manager is to be responsible for the overall coordination of services to meet the goals of the clientfamilies focused plan of care
2. This is accomplished by building relationships with the caregivers and care team and acting as a liaison to other agencies and ensuring they receive all necessary services and implementing the plan of care
3. Our aim is to provide the highest quality of care management services by complying with regulatory guidelines and meeting agency standards
4. Assists families to plan and obtain services appropriate to the needs of the child and family
5. Completes the Child and Adolescent Needs Assessment CANS to identify childs medical social educational and psychological needs
6. Then develops a comprehensive plan of care based on the identified needs
7. Coordinate and monitor the delivery of identified services and the proper use of resources
8. Deliver at least 2 client centered health home services to each client on a monthly basis
9. Attends and participates in continuing education ongoing staff in services and quarterly meetings
10. Responding to feedback and corrections from company quality assurance in a timely fashion
11. Perform related duties as assigned by the Supervisor
12. Maintain compliance with all company policies and procedures
13. Other job related duties as assigned

Education andor Work Experience Requirements

1. Bachelors degree is required
2. preferred fields of study are Psychology Sociology Social Work or Human Services; or a minimum of 2 years of job related experience providing case management or medical focused care coordination services to individuals with special needs
3. Must be in person in our Manhattan or meeting with clients
4. Each case requires two monthly in home visits
5. Must attend all weekly in person staff meetings
6. Excellent verbal and written communication skills including ability to effectively communicate with clients and healthcare providers
7. Need to use electronic health record systems databases and other computer based tools to manage client information and maintain accurate records
8. Strong organizational skills to help manage multi family case load
9. Willingness to meet with families in their homes or communities
10. Bilingual in Mandarin is required
11. Cantonese is a plus
12. Computer proficiency MS Office Word Excel and Outlook
13. Valid NYS issued ID or drivers license; must be willing to travel throughout the 5 boros of New York City
14. Ability to work independently and to carry out assignments to completion within the parameters of instructions given prescribed routines and standard accepted practices

Physical Requirements

1. Ability to travel to numerous locations to meet with clients attend meetings and visit healthcare facilities
2. Ability to safely and successfully perform the essential job functions consistent with the ADA FMLA and other federal state and local standards including meeting qualitative andor quantitative productivity standards
3. Ability to maintain regular punctual attendance consistent with the ADA FMLA and other federal state and local standards
4. Must be able to lift and carry up to 35 lbs
5. Must be able to talk listen and speak clearly
