Tri County Care Management Organization (CMO) is a non-profit organization that provides care management (case management) services for children with complex behavioral, developmental or addiction challenges in Hunterdon, Somerset and Warren counties. We are currently seeking a Care Manager based in our Stewartsville office to provide care management (case management) services.

Responsibilities Include:

• Provide intensive wraparound/case management services to families and children with mental health/ behavioral/developmental challenges.
• Advocate and help find resources for youth to access formal and informal services and be a liaison for care services.
• Attend meetings at homes, schools and other services with the youth and families.
• Document and follow up on plans of care.

Qualifications

Position Requirements:

• Bachelor’s degree in social work, psychology or related discipline
• At least one (1) year of experience in a child service system environment
• Experience with DD/ID population preferred
• Strong interpersonal and organizational skills
