Job summary

St. Francis Hospital & Heart Center in Roslyn, NY is seeking a Care Manager RN to lead patient care across the acute care continuum, building strong relationships with patients, families, and the care team. Responsibilities

• Assesses, plans, implements, monitors and evaluates options and services to effect an appropriate individualized plan of care for patients across the acute care continuum using independent judgment and discretion.

Requirement

• NYS RN License
• B.S. in Nursing3-5 yrs.
• BLS certified with the American Heart Association
• Recent med/surg or cardiac experience, PRI and/or case management certification preferred.

Salary Range

USD $49.65 - USD $74.76 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
