Job summary

Home Helpers Home Care of Manassas-Gainesville is seeking a compassionate, organized, and motivated Care Manager to join our growing team. You will coordinate caregiver schedules, assist with onboarding, and develop and maintain care plans to ensure high-quality, compliant home care for seniors and families.

You will collaborate with nurses, clients, and family members, manage WellSky documentation, and uphold HIPAA standards while providing outstanding support and caregiver guidance.

Qualifications

• High School Diploma or GED required.
• AA/BA in Healthcare Admin or Nursing preferred.
• Experience in home care, healthcare, or care coordination preferred.
• Strong organizational and communication skills.
• Proficient with Microsoft Office and Outlook.
• Valid driver’s license and reliable transportation.

Responsibilities

• Coordinate caregiver schedules and client assignments.
• Assist with onboarding and training new caregivers.
• Coordinate client admissions and nursing assessments.

• Monitor WellSky documentation and compliance.
• Maintain accurate client and caregiver records.
