Job Description

As a RN Care Manager (RNCM), you will work collaboratively as part of a care team to assess consumer needs and establish a person-centered plan of care. Your responsibilities will include monitoring the daily health and well-being of our consumers through technology, information from caregivers, and conducting monthly home visits. Specific duties include:

1. Coaching, educating, and supporting family caregivers
2. Developing person-centered plans of care
3. Using structured assessment tools to evaluate consumer health status
4. Conducting regularly scheduled home visits
5. Representing Caregiver Homes professionally in the community

Qualifications

Qualified candidates must be compassionate, capable of building strong relationships with families, able to work collaboratively, solve problems, and be flexible and proficient with technology. Candidates must hold an active RN license and have at least two years of relevant experience as a registered nurse, preferably with elders, individuals with disabilities, and those with complex medical needs in healthcare, long-term care, social services, or community settings. Willingness and ability to travel to serve our caregivers and consumers in their homes are required. Valid driver’s license is also necessary.
