Job summary

AdventHealth in Burleson, Texas is seeking an experienced RN to lead discharge planning and social services across inpatient, observation, and ED settings. You will drive care management strategies and collaborate with facility leadership to improve transitions.

You will participate in rounds, mentor staff, review documentation with dashboards, and coordinate complex transitions. A strong background in CMS rules, medical necessity, and interdisciplinary teamwork is essential.

Qualifications

• 1+ year in direct care management
• 2+ years frontline nursing
• RN license required
• ACM/CCM preferred

Responsibilities

• Supervise discharge planning and social services operations.
• Implement corporate care management strategies with facility management.
• Participate in multidisciplinary rounds to ensure effective discharge planning.
• Provide expert discharge planning and progression of care.
• Use reports and dashboards to monitor care manager documentation.
• Facilitate care conferences for complex transitions.
• Participate in meetings to review high-cost or difficult placements.
• Report quality and risk issues promptly.
• Address complaints and follow up with timely communications.
• Participate in hiring, onboarding, training, evaluating, mentoring staff.
• Perform other duties as assigned.

Job Description:

• Supervises daily operations of discharge planning and social services in the inpatient, observation, and emergency department, serving as the primary point of contact for inpatient unit Care Management barriers and concerns.
• Implements corporate Care Management strategies in collaboration with facility management.
• Actively participates in multidisciplinary rounds, ensuring active staff participation and effective discharge planning and barrier resolutions.
• Provides discharge planning and progression of care expertise to the interdisciplinary team.
• Leverages reports and dashboards to monitor the accuracy of care manager team's documentation and compliance with regulations.
• Facilitates care conferences for complex transitions and placements as needed.
• Participates in hospital and medical staff meetings to review patients with high dollar, extended length of stay, or difficult placement.
• Reports pertinent quality and risk issues to appropriate individuals, departments, and committees in a timely manner.
• Addresses patient, staff, physician, and department complaints and follows up with timely communications and escalations.
• Participates in hiring, onboarding, educating, evaluating, and mentoring staff.
• Performs other duties as assigned.
