Job summary

UCHealth Med Ctr of the Rockies in Loveland, CO is seeking a Social Worker for the MCR Care Management team. This onsite role focuses on assessing and planning interventions to support patients with social, emotional, economic, and environmental needs.

Full-time, days, 80 hours per pay period, salary range $31.61-$47.41 per hour. Requirements include a Master’s degree in Social Work and state licensure as LSW or SWC.

Qualifications

• Master's degree in Social Work required.
• State licensure as Licensed Social Worker (LSW) or Social Work Candidate (SWC) required.

Responsibilities

• Interviews and assesses patients and/or patient’s family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Assists with discharge planning and processes; identifies resources and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Ability to think critically, work with minimal supervision, and perform in a fast-paced environment.
