Job description

Our client, a leading hospital system, is seeking a Care Management Manager (RN or MSW) to lead their Care Management Department at their Fullerton, CA medical center. This is a full-time, onsite leadership role ideal for an experienced healthcare professional with strong clinical, operational, and leadership expertise.

Reporting to the Director of Care Management, the Manager will be responsible for the day-to-day operations and clinical effectiveness of care coordination across the continuum. You will oversee a team of Registered Nurses, Social Workers, and support staff, while ensuring the delivery of high-quality, patient-centered care that supports optimal health outcomes and effective resource utilization.

Key Responsibilities:

  • Direct daily operations of the Care Management Department, ensuring regulatory compliance and patient-focused care.
  • Supervise, mentor, and evaluate a multidisciplinary team of RNs, MSWs, and care support staff.
  • Facilitate discharge planning and care transitions by collaborating with clinical and non-clinical teams.
  • Monitor and promote compliance with hospital, state, and federal standards.
  • Lead the implementation of departmental goals, projects, and care coordination initiatives.

Salary & Benefits:

  • Base Salary Range: $113,000 – $178,000 annually
  • Additional compensation may include:
    • Shift differentials
    • Overtime and standby pay
    • On-call premiums

Qualifications:

Required:

  • Bachelor’s in Nursing (BSN) or Master’s in Social Work (MSW)
  • Minimum 3 years of experience in an acute care or ambulatory setting
  • At least 1 year of recent leadership experience managing clinical staff
  • Proven experience with project management and healthcare operations

Preferred:

  • Master’s in Nursing, Healthcare Administration, or related field
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM)
  • 3+ years of healthcare management experience
