Corewell Health is seeking a Care Management Manager to oversee coordination of inpatient care, utilization management, and related activities at Royal Oak Hospital. You will lead a multidisciplinary team, work under the Care Management Director, and ensure patients receive appropriate care in the right setting while optimizing resources.

Responsibilities include directing activities in one or more key functions, collaborating with multiple disciplines, and reporting to the Care Management

Qualifications

• Must hold a BSN or MSW with relevant licensure.
• Minimum 5 years of nursing or social work experience.
• 3 years of acute care management experience preferred.
• MI RN license or MI MSW credential required.

Responsibilities

• Coordinate activities across inpatient care management, EC care management, and utilization management.
• Lead and develop staff within the Care Management Department.
• Collaborate with multiple disciplines to expedite care coordination.
• Report to the Care Management Director and ensure optimal patient care and resource use.
