Job Description: CFSP Care Management Manager

We are partnering with North Carolina DHHS to operationalize a statewide Medicaid Plan designed to support Medicaid-enrolled infants, children, youth, young adults, and families served by the child welfare system, ensuring they receive seamless, integrated, and coordinated healthcare. Within the Children and Families Specialty Plan (CFSP), members will have access to benefits including physical health, behavioral health, pharmacy, intellectual/developmental disabilities (I/DD) services, long-term services and supports, unmet health-related resource needs, and integrated care management. Our vision is for a North Carolina where all children and families thrive in safe, stable, and nurturing homes.

Note: North Carolina residency is required.

Location & Work Environment

This is a field position. When not in the field, you will work remotely from your home. You must reside within 20 miles of Pitt County.

Hours: General business hours, Monday through Friday.

Travel: Some travel within your assigned area or region is required.

Position Overview

The CFSP Care Management Manager (Manager I GBD Special Programs) is responsible for managing assigned care managers and ensuring fidelity to the CFSP Care Management model, including physical health, behavioral health, and social services.

Primary Responsibilities
  1. Manage resource utilization to ensure appropriate care delivery and adequate coverage for all tasks.
  2. Review all care plans and ISPs for quality control and guide care managers in addressing members’ complex health and social needs.
  3. Ensure care managers provide trauma-informed care, recognizing the impact of ACEs.
  4. Coordinate service delivery, including member assessment of physical and psychological factors.
  5. Participate in cross-functional workgroups to maintain and develop the program.
  6. Evaluate current processes and recommend improvements for efficiency and outcomes.
  7. Develop and conduct staff training programs.
  8. Analyze data to present findings to stakeholders.
  9. Hire, train, coach, evaluate, and manage performance of direct reports.
Minimum Requirements
  • Active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by North Carolina.
  • MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing.
  • At least three (3) years of experience in care management, case management, or care coordination within the child welfare system.
Preferred Qualifications
  • Knowledge of resources and services for safe community living, including LTSS, behavioral health, and physical health services.
  • Experience working with children, youth, and families served by local DSS through foster care and adoptive assistance programs.
  • Minimum two (2) years of management/supervisor experience with direct reports.
  • Experience in service delivery coordination, discharge planning, or behavioral health in a managed care setting.
