Job summary

Alexander Youth Network in Greensboro, NC is seeking a Care Management Support Specialist to assist Tailored Care Managers by providing information about services and connecting clients with community resources to improve health outcomes.

You will also help engage Tailored Care Management clients and participate in in-person activities as needed to support individuals enrolled in the program. A high school diploma and experience with children's mental health are required.

Qualifications

• High school diploma or equivalent.
• Two years of paid experience working directly with children’s mental health.
• General outreach and Tailored Care Management experience.
• Experience with a behavioral health condition or has served as Certified Peer Support Specialist.
• Valid NC or SC Driver's license.

Responsibilities

• Support clients by providing information about accessing services and community resources to improve health outcomes.
• Engage Tailored Care Management clients and perform in-person activities as needed to support individuals enrolled in Tailored Care Management.

What You’ll Do:

The Care Management Support Specialist works under the supervision of a Tailored Care Manager, supporting clients by providing information about and assistance with accessing services and community resources to improve the individual's health outcomes. The Care Management Support Specialist also supports engagement of Tailored Care Management clients and performs in-person activities as needed to adequately support the needs of individuals enrolled in Tailored Care Management.

Qualifications: Education and Experience
