Springpoint at Home is seeking an experienced Care Management Coordinator/Aging Life Care Advisor (TM) for the Mercer, Middlesex & Essex County area to join our team!

Springpoint at Home offers home care and care management services to residents of our Life Plan Communities as well as to individuals living in their private homes. We are seeking a full-time Care Management Coordinator/Aging Life Care Advisor to join our team. The CMC is a health and human services professional who acts as a guide and advocate for clients and families, focusing on aging and elder care issues. The role involves daily care management activities, monitoring, and chart audits of part-time care managers.

Responsibilities:

  • Market and promote all SAH services, emphasizing Care Management/Aging Life Care Advisor (TM).
  • Promote client psychosocial wellbeing and self-determination.
  • Coordinate with healthcare providers, family members, trust officers, accountants, and support networks.
  • Arrange transportation for medical appointments, social events, and outings.
  • Maintain confidentiality of all client information.
  • Perform duties per Springpoint at Home policies and procedures.
  • Complete assessments, reports, and documentation, filing all appropriately.
  • Communicate client status changes during weekly meetings and as needed.
  • Provide updates on POA/Guardian/Conservator at least monthly or as required.
  • Maintain client files with EHR documentation.
  • Assist with scheduling and notification of care aides.
  • Participate in interviewing care management staff.
  • Generate reports for the Director and Nursing Supervisor as needed.

Education and Experience:

  • Graduate degree in social work, gerontology, or equivalent experience qualifying for certification as a Care Manager and ALCA membership.
  • 2-4 years of geriatric care management or healthcare coordination experience.
  • Proficiency with office equipment and basic business functions.

Pay Range: $34.00 - $40.00/hr
