We offer a broad range of services including cardiovascular health, behavioral health, allergy testing, in-house lab testing, imaging, chronic disease management, and other specialties. We are seeking a Care Coordinator to monitor patient care and support them in achieving health goals, coordinating with providers to meet patient needs. A clinical knowledge background is preferred.

Chronic Care Management (CCM)

CCM is a healthcare model providing comprehensive care for patients with chronic conditions, aiming to improve care quality, patient outcomes, and reduce costs by addressing complex health needs.

Care Coordinator Responsibilities

1. Develop personalized care plans with patients, caregivers, and healthcare providers.
2. Coordinate communication among healthcare team members.
3. Monitor adherence to treatment plans and progress towards health goals.
4. Provide education and support on disease management and preventive care.
5. Use electronic health records (EHR) and healthcare technology to document and track care.
6. Work with community resources to address social determinants of health.
7. Conduct follow-ups and care activities to optimize health outcomes.

Qualifications

• Minimum 1 year of clinical experience as a Medical Assistant.
