Job description

Care Management Associate - OhioRISE (Must live in Ohio)

At CVS Health, we’re building a world of health around every consumer and surrounding ourselves with dedicated colleagues who are passionate about transforming health care.

As the nation’s leading health solutions company, we reach millions of Americans through our local presence, digital channels, and more than 300,000 purpose-driven colleagues – caring for people where, when, and how they choose in a way that is uniquely more connected, more convenient, and more compassionate. And we do it all with heart, each and every day.

Position Summary

OhioRISE manages the behavioral health benefit for eligible children and young adults receiving Medicaid services in Ohio. The CMA will regularly interface with various groups around the state who are providing care coordination services or supporting the services utilized by our members. This includes, but is not limited to, care coordinators, managers, community members, referral sources, children’s services, guardians, family members, case managers, and behavioral health providers.

This role acts as the 'front desk' for the OhioRISE plan, handling inbound care management inquiries from members, guardians, or service providers. Responsibilities include taking referrals, confirming Medicaid eligibility, processing referrals, and completing Health Risk Assessments for assigned members. The CMA must demonstrate professionalism and compassion when interacting with departments and member representatives. Additionally, the role requires supporting administrative tasks, working with complex data systems, electronic record systems, and Excel. Tasks may change with little notice to meet the health plan’s needs, requiring flexibility.

This is a full-time remote telework position. Qualified candidates must reside in Ohio. The schedule is Monday – Friday, 8 am - 5 pm, with no holidays or weekends. Candidates need a private, distraction-free workspace and must secure their own internet; equipment will be provided prior to start.

Required Qualifications
  • Recent experience (within the last 5-7 years) working for other MCOs in Ohio, or recent employment with healthcare organizations.
  • Ability to pass and obtain CANS assessments certification within 90 days of employment.
  • Ability to handle escalated calls, problem-solve in stressful situations, multitask across multiple systems, take initiative in learning, and contribute positively to the work environment.
Preferred Qualifications
  • Associate’s degree or other healthcare certifications or training.
  • Interest and desire to help individuals with behavioral health needs.
Education
