Hampshire County Council is seeking Care Leavers Personal Advisors to support young people aged 16–25 as they transition to adulthood. You will mentor, coordinate visits, and work with partner agencies to enable positive outcomes in a hybrid working environment.

You will manage caseloads, maintain records, and contribute to a supportive, inclusive team focused on empowering care leavers to achieve independence and success.

Qualificações

• Ability to establish and maintain relationships with young people aged 16-25 and work with partner agencies and families/carers.
• Understanding of adverse childhood experiences and challenges faced by care leavers transitioning to adulthood.
• Demonstrate patience, empathy, and compassion in all interactions.
• Team player with collaboration skills and ability to lone work with risk assessment.
• Sound IT skills and accurate recording of information.

Responsabilidades

• Manage a caseload of care leavers aged 16–25 from diverse backgrounds.
• Organise diary to prioritise visits and complete administrative tasks.
• Act as mentor and coach to help young people reach their potential.
• Liaise with partner agencies to achieve best outcomes for young people.
• Work in a hybrid setting, visiting locations, home, and offices.
