Job summary

Providence Medical Foundation is hiring a Care Management support specialist who assists the Care Management team in coordinating patient care across the healthcare continuum. The role works closely with the healthcare team and performs clerical tasks identified during initial and ongoing assessments.

Qualifications

• Medical Terminology coursework or working knowledge.
• 1 year healthcare experience.

• Proficient in Microsoft Office (Word & Excel).
• Bilingual English/Spanish communication skills desired.

Under the direction of the Supervisor or Manager, this position is responsible for providing support to the Care Management team who coordinates care for identified members/patients throughout the healthcare continuum. This position works closely as a healthcare team member and performs clerical tasks related to the overall team functions and activities identified during the initial and ongoing assessment and management of members/patients.

Required Qualifications

• Coursework/Training: Medical Terminology coursework or Working knowledge of Medical Terminology.
• 1 year Healthcare.

Preferred Qualifications

• Associate's Degree In a related field.
• Upon hire: Medical Assistant Certification (CMA)
• Proficient in all Microsoft Office applications including Word and Excel.
• Bilingual English/Spanish communication skills.
• Managed care or case management.Experience with Mcg Care Guidelines, Touchwork, IDX, Meditech.
