Care Management is a collaborative practice model including the patients, nurses, Care Manager LCSWs, physicians, other practitioners, caregivers and the community. Required Qualifications:

• Master's degree in social work
• Upon hire: California Clinical Social Worker License
• Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
• 2 years of Relevant Social Work experience in a medical group, health plan, hospital, community, post-acute, or medical office/clinic setting.

Preferred Qualifications:

• 3 years of Care management and Managed Care experience in acute, ambulatory, or community health setting.
• 3 years of Behavioral Health experience in acute or outpatient setting.
